ADHD, Anxiety, and Depression Evaluations
If you suspect that your child may have ADHD/ADD, anxiety, and/or depression, please feel free to take this online asynchronous assessment and you'll receive the results very quickly, usually within 3 days or less! If the doctor or psychologist determines that your child has a diagnosis, you will be able to use the report as proof of eligibility for the Step Up For Students FES-UA Scholarship. The evaluation is not done by JoAn Stead, but by Mentavi.

If you need a Florida-licensed psychologist for the FES-UA Scholarship, please TEXT JoAn at 850-723-4151 and wait for a confirmation. If you submit the assessment to Mentavi without getting a confirmation from JoAn, then you submit it at your own risk and may not receive an evaluation by a Florida-licensed psychologist and may incur additional fees if you have to ask Mentavi to re-assign your assessment.
