Asynchronous Virtual Homeschool Evaluation

This type of homeschool evaluation does not require an appointment because the parent will communicate asynchronously via chat, text, email, etc. and submit this online form when ready to begin the process.

Step 1: Fill out this intake form for each child.
Step 2: Submit Payment Online or if using Step Up For Students Scholarship--use coupon code: scholarship2 on the payment page.
Step 3: Send Photos and/or Video Clips of your child's evidence of progress--this can include pics of workbooks, tests, online grades, PERT scores, standardized test scores, FLVS grades, Dual Enrollment grades, video clips flipping through school work from each subject, video clips of your child reading, video clips of your child doing learning activities, photos of hands-on activities, photos of projects, etc.

How will you send your child's evidence of progress?

(Examples of evidence of progress includes grades or reports from online courses, 3-5 photos of samples of work from each subject, video clips flipping through samples of school work, a 30-second video or audio clip of your child reading, test results, etc.) If emailing samples of work, grades, etc. please send to: EvalsByHomeschoolNest@gmail.com

If your child has any special needs that you would like to share about, please feel free to mention here. (Autism, Speech/Language, Dyslexia, ADHD, etc.) This info will remain private and is only taken into consideration for the evaluation.

If you have multiple children needing homeschool evaluations, a separate intake form will need to be filled out for each child.

When is your homeschool evaluation due by? (Your annual due date is the date that you signed your Letter of Intent.)
